For free admission to the Centre’s dialysis treatment:

• Contact the Centre (by phone, fax, email or the Information Request Form) specifying the period and number of treatments desired.

• Send us your Registration Form or a Dialysis Form issued by your own Dialysis Centre, complete with recent blood results with screening of viral markers for HBV, HCV and HIV. These should be no more than 3 months old.

• Dialysis treatment free of charge: on presentation of your European Health Insurance Card (please send a photocopy with your registration form)
